Checkboxes for all fields on Liability Waiver

Additional to my request for the checkboxes for YES or NO answers to questions, i would like to have the option to include checkboxes for every other question where it says SIGN right now as well. 

This serves as a double confirmation that people NEED to check all the boxes before they can sign. This is another check to confirm that people have read all the information they are signing for.
