It asks for Certification level and leaves it to the client to type it in. I think this should be a drop down menu we cert levels the dive center verifies. Like for my existing form we have -

1. No Certification

2. Open Water

3. Advanced Open Water

4. Rescue

5. Professional

6. Cave / Tech

Dont really need anything other than those, but some shops may want more or less detail.
