Merge Forms?

I am still brainstorming how to increae my rate of completion for waivers. Right now its still really low, especially on secondary divers.

I am thinking if there is a way to merge forms “Standard Profile” and “ Equipment Rental Release” and “Liability Waiver”. I am wondering if have it appear as one block to them if under profile if it will increase our completion percentage.

I tried just not adding a new PDF to the “Standard Profile” form but it was giving me errors. I was thinking of just adding onto the profile block with the two waivers and single signature for all 3 blocks. I had the errror which denied adding the PDF, but besides this I think if I add them in this manner I would then lose my notifications for incomplete forms wouldnt I?
